What documents should I attach?

If your account was registered in the name of an individual person, you will need to provide a color copy of that individual’s Drivers License or State ID.

If the account had a company name listed during registration, you will need to submit a copy of one of the following documents as proof of ownership.

  • Business License
  • Certificate of Association
  • 501(c)(3)
  • DBA/ Fictitious Name Documentation

What information should be on the documents?

  • Individual/Company Name
  • Street Address matching the address on the account
